Change of Surname (Maiden to Married Name)

Individuals wishing to change their surname after marriage must provide:

• Completed Civil Registration Affidavit Form CRVS3(a).
• Certified Marriage Clearance Form from the maiden Umphakatsi (Royal Kraal).
• Certified Marriage Certificate.
• Certified copies of both spouses’ National ID Cards and Passports.
• Application letter requesting the change of surname.
